The Department of Human Resources has prepared form 1095-C, Employer-Provided Health Insurance Offer and Coverage, for the calendar year 2018, as required by the Affordable Care Act. U.S. taxpayers must prove that they had health insurance coverage in 2018 when filing their taxes for 2018. This year, the IRS requires that the 1095-C forms be issued to employees by March 4, 2019.

Similar to last year, we have begun delivering the 1095-C forms to the schools and departments for distribution to employees this week. To protect the privacy of our employees, please distribute 1095-C form (sealed form) only to the employee named on the form. If an employee is  unable to get their 1095-C in person, please send these forms back to Human Resources immediately so that they can be mailed to their address on record.
