Do you want to adopt a healthier lifestyle? Do you need some support along the way?<\/p>\n<p>As part of our commitment to health and wellness, the Real Appeal health and wellness benefit is available to members of the UnitedHealthcare insurance plan.<\/p>\n<p>The free, step-by-step, personalized wellness program helps individuals lose weight and lower their risk for chronic diseases such as heart disease, Type 2 diabetes and more.<\/p>\n<p>Employees and their spouses and dependents 18 and older who have ACPS UnitedHealthcare insurance and a body mass index (BMI) of 23 or higher can participate in the program <b>at no cost<\/b>.<\/p>\n<p>The year-long program includes:<\/p>\n<ul>\n<li>a personal transformation coach<\/li>\n<li>an online portal with resources<\/li>\n<li>mobile app integration<\/li>\n<li>an online support group<\/li>\n<\/ul>\n<p>The Real Appeal Success Kit, delivered to your door, includes:<\/p>\n<ul>\n<li>guides<\/li>\n<li>fitness DVDs<\/li>\n<li>a personal blender<\/li>\n<li>cooking tools<\/li>\n<li>a scale and more<\/li>\n<\/ul>\n<p>The program offers everything you need to kickstart your weight loss and help you develop healthy habits.<\/p>\n<p>If you have already enrolled and would like to become an active member, contact the Real Appeal Answer Center at 1-844-344-REAL (7325).
