This information will assist schools with contact tracing and prevent any unnecessary quarantining of students who have been vaccinated.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">This information is voluntary at this time. Families who are interested should complete the <\/span><a href=\"https:\/\/acpsva.qualtrics.com\/jfe\/form\/SV_bK04oNEpvXNB4Kq\"><span style=\"font-weight: 400;\">ACPS Student COVID-19 Vaccine Documentation Form<\/span><\/a><span style=\"font-weight: 400;\">, which requires one form per student.
